Early Variations in Augmentation Index and Pulsed Wave Velocity in Patients with Obstructive Sleep Apnea and Arterial Hypertension Post CPAP Treatment

چکیده

Obstructive sleep apnea (OSA) is a chronic repeated upperairway obstruction during sleep, characterized by repetitive apnea, hyperpnoea and oxygen desaturations leading to interrupted sleep, variations in blood pressure, and increased sympathetic nervous system activity [1]. The prevalence of OSA is 2–4 % in the general population [2-4]. The prevalence of cardiovascular disease (systemic hypertension, coronary artery disease and cerebrovascular disease) was high in OSA patients [5]. The number of OSA patients with hypertension is high (35 %) to (70 %) [6,7] and nearly 30 % of patients presenting with hypertension also have OSA [8]. OSA has been proven to be a causal factor in the pathogenesis of vascular dysfunction and hypertension [9]. The Noninvasive assessment of arterial stiffness by the measurement of pulsed wave velocity (PWV) and augmentation index is a predictor of cardiovascular morbidity and mortality in hypertensive patients [10].The Continuous positive airway pressure (CPAP), is considered the first line treatment for OSA and can decrease the arterial stiffness in hypertensive patients with obstructive sleep apnea [11,12]. Effects of CPAP on “vascular” risk factors in patients with obstructive sleep apnea and arterial hypertension

BACKGROUND The aim of this study was to assess the effects of continuous positive airway pressure (CPAP) on arterial stiffness, central blood pressure, and reflected pulse wave characteristics in patients with severe obstructive sleep apnea (OSA) and stage 2-3 arterial hypertension. METHODS Forty-four patients with hypertension and severe OSA (apnea/hypopnea index > 30) received stepped dose ...

Treatment of Sleep Apnea With CPAP Lowers Central and Peripheral Blood Pressure Independent of the Time-of-Day: A Randomized Controlled Study.

BACKGROUND Obstructive sleep apnea (OSA) is a risk factor for hypertension and randomized controlled trials have shown that OSA treatment with continuous positive airway pressure (CPAP) reduces peripheral blood pressure and arterial stiffness. Arterial stiffness is known to augment central aortic blood pressure independent of peripheral brachial blood pressure. Currently, it is unclear whether ...

Factors Affecting Patients’ Adherence to Continuous Positive Airway Pressure Therapy for Obstructive Sleep Apnea Disorder: A Multi-Method Approach

Background: Continuous positive airway pressure (CPAP) is the gold standard therapy for treating obstructive sleep apnea (OSA) disorder. However, patients’ adherence to its regular use is poor. The present study aimed to determine the adherence rate to CPAP therapy by identifying factors affecting its regular use and its associated problems and discomforts among a sample population in Mashhad (...

Effects of Biofeedback Therapy on Cardiovascular and Respiratory Indices in NREM Sleep Parasomnias with Obstructive Sleep Apnea: A Case study

Introduction: Non-REM parasomnias are a relatively common condition in the general human population. Current treatment plans are usually based on small case series and reports. Considering the effects of sleep disorders on different aspects of human life and the failure of pharmacological treatments in this field, the present study was designed to investigate a case of this sleep disorders with...
